The director was cooking lunch while having a 4-month-old in her care in a playpen outside of the kitchen.

An incident/illness report was not filled out for the parent to sign. A handwritten note was used in its place.

A child was left on the playground without supervision for approximately 15 minutes.

During the monitoring visit the playground had trash accessible to children, the grass is needing to be mowed and the black tarp adding privacy is falling off of the fence causing a hazard.

The video footage shows the caregiver did not account for all of the children exiting the transportation vehicle which resulted in a child being left unattended for approximately 7 minutes.

Video shows a caregiver grabbing a two-year-old child's arm & pulling them down to the ground. Another video shows the caregiver grabbing a different 2-year-old child and pulling them to the ground to sit in time out in front of a shelf.

In the two-year-old classroom, two children were placed in time out for an extended period. One was in time out for 6-7 minutes and the other was in time out for approximately 10 minutes.

Based on the information gathered during the investigation, a caregiver grabbed a child by the arm and hit the child on the back.

There was an epi pen that was expired. This was corrected when the director set it aside to give to the parent.

The Children's Product Certification was last signed on 7-10-2023. This was corrected when the director signed the form and sign up for the emails.

A caregiver was observed handling a toddler roughly to turn child over onto their back so they can remain lying on the mat. Child was still fidgeting and crying, then child was lifted by the caregiver from their mat and sat firmly on an adult chair next to her mat.

The last annual fire inspection was conducted on 7/5/2023. The next fire inspection is scheduled for 7/18/24. This request to schedule it was made today, 7/16/24.

Three of nine children's files reviewed did not include an up-to-date immunization record.

One of nine children's files reviewed did not contain a signed emergency medical authorization.

An infant caregiver was observed changing an infant's diaper without keeping a hand on the infant. Caregiver turned away from the infant to grab supplies off a high shelf behind the changing table.
