The Franchise Tax has ended involuntarily.

During the investigation inspection, 4 infants were observed to be unsupervised.

During the inspection, 9 out of 9 files did not have proof of background check available for review. 1 staff out of 9 files viewed did not have their training for calendar year, 2024-2025. 4 out of 9 staff files viewed did not have their discipline and guidance paperwork signed available for review. 2 out of 9 staff files did not have their notarized affidavit (Form 2985) available for review. 1 out of 9 staff files did not have their preemployment affidavit (Form 2912) available for review. . 3 out of 9 staff files viewed did not have their Child Care Personnel Information (Form 2947) available for review.

During the inspection, 1 out 9 files caregiver did not have current CPR/First Aid available for review. It expired on 1/14/2025.

The operational polices were viewed during the inspection and the following were missing: 17) Animals, if applicable (19) Procedures for providing and applying, as needed, insect repellent and sunscreen, including what types will be used, if applicable (20) Parent rights that are consistent with the rules in Division 5 of this subchapter (relating to Parent Rights) (23) Instructions on how a parent may access the [Medium]: (A) Minimum standards online; (B) Texas Abuse and Neglect Hotline; and (C) HHSC website. (26) Preventing and responding to abuse and neglect of children, including: (A) Required annual training for employees [Medium]; (B) Methods for increasing employee and parent awareness of issues regarding child abuse and neglect, including warning signs that a child may be a victim of abuse or neglect and factors indicating a child is at risk for abuse or neglect [Medium]; (C) Methods for increasing employee and parent awareness of prevention techniques for child abuse and neglect [Medium]; (D) Strategies for coordination between the center and appropriate community organizations [Medium]; and (E) Actions that the parent of a child who is a victim of abuse or neglect should take to obtain assistance and intervention, including procedures for reporting child abuse or neglect 29) If your operation maintains and administers unassigned epinephrine auto- injectors to use when a child in care has an emergency anaphylaxis reaction, policies for maintenance, administration, and disposal of unassigned epinephrine auto-injectors that comply with the unassigned epinephrine auto- injector requirements set by the Texas Department of State Health Services, as specified in 25 TAC Chapter 40, Subchapter C (relating to Epinephrine Auto- Injector Policies in Youth Facilities) and in Texas Health and Safety Code ?773.0145.

A review of the Employees Who Left Employment in 2024 Survey data from January 17, 2025, revealed that the center had not reported the number of employees who left employment in 2024 by January 15, 2025, as required.

There were five infants under the age of 12 months old with one caregiver. Ratio was not in compliance. Note: An extra caregiver was working in the infant room immediately.

There were five roosters in the outdoor play area accessible to children in care. Note: The roosters were removed from the play area during the inspection.

Two infant bottles were sitting on the counter. The bottles were not labeled.

The annual fire inspection has not been conducted. The last fire inspection was conducted on 4/5/2022.

Cribs in the infant room were lacking the required name labels. SN: This was corrected during inspection.

A changing mat located in the toddler room was observed to be torn. SN: This was corrected during inspection.

The loose-fill did not meet the required depth causing the plastic lining to show.
