There were multiple hazards on the outside play area. There were two refrigerators that children had access to that they could become trapped inside. There were broken pieces of a fan that had electrical cords and sharp blades.

The emergency preparedness plan was not complete and available for review.

The emergency practices form was not current with a fire drill, checks of the carbon monoxide detector, checks of the fire extinguisher and checks of the smoke detectors.

The primary caregiver did not have current trainings available for review.

There was not a current rabies vaccination report for the dog.

The permit holder was operating as a licensed childcare center without a permit or applicaiton on file.

Three staff members were present at the opeation without initial background checks.

The licensed childcare home was over capacity by five children.

The operation was out of compliance when there were more children present in the home at the same time than allowed by minimum standards. There were a total of 17 children in care at the same time.

One caregiver counted in ratio did not have a current CPR/First Aid on file.
