When reviewing staff files it was found that one staff member did not have a current CPR/First-Aid certificate.

Four staff files were reviewed and one staff file was missing 20 training hours.

During the walk through, three classrooms visited the caregivers did not know the correct number of children they were responsible for.

During the walkthrrough 5 panels were observed outside (Panels that hold the building structure) is decaying/rotting and is not properly holding the building post. Note: Children walk/play in this area.

It was observed that the unitary surfacing is breaking/wearing off from the edges in multiple areas.

Two out of ten personnel records reviewed is missing the education qualifications. Note: The graduation date is not documented on the application.

It was observed that multiple areas in the play structure is rusting causing the rubber to break which creates a sharp surface.

It was found that the caregiver did not know the number of the children the caregiver was responsible for. The caregiver also did not do name to face before going outside and left a child inside for 8 minutes.

It was found that a child was left unsupervised inside the classroom alone for 8 minutes.
