The operation did not have activity plans postedfor each age group.

Two of the four children files reviewed did not have health care statements.

The operation did not have nap mats or cots at the operation during the inspection.

During the inspection it was observed that the transportation bus was missing tweezers and scissors and the antiseptic wipes were expired. This was corrected at inspection when the supplies were all replaced.

During the inspection it was observed that a fire drill had not been practiced since May 2024. This was corrected at inspection when a drill was conducted while I was present.

The last time the employee list was validated was on 10/11/23.

The after schoolers were watching You Tube programming not related to educational needs. The program was changed when this was brought to the attention of the caregiver. A television program called Go Dan was put on where children learn about letters and numbers by dancing.

The last Fire inspection was conducted on 7/30/2021.

It was found that two out of ten caregivers files reviewed did not have the renewed transportation training. Note: This training is scheduled for Saturday

A review of the Employees Who Left Employment in 2021 data from February 23, 2022, revealed that the center had not reported the number of employees who left employment in 2021 by February 22, 2022, as required.

During the follow up inspection two caregivers were present with pending background checks. Note: Both caregivers left the operation.

During the investigation inspection it was found that the infant room is out of ratio by one child. One caregiver had 5 infants in care (0-11 month classroom).
