In one classroom, observed inadequate space between each nap mat/cot.

The outdoor play area bumpers around the loose fill has 3 nails sticking up out of them while children were present outside. This was corrected when the director hammered them back down during inspection.

One staff did not have handling, storing, and disposing of hazardous materials training.

One tile in a children's bathroom is cracked and raised up off of the floor.

Two caregivers did not have training certificates for Sids/Shaken baby, and Infant Brain Development trainings that were completed.

Two caregivers hired after 9/1/23 did not have the Pre-Employment Affidavit form 2912.

The outdoor area had plastic bumpers around the loosefill that had four different areas with cracks in them while children were present.

An individual was present at the operation without an eligible background check during inspection. The individual left the operation during the inspection.

Two caregivers did not have information showing each child's age that they was responsible for. Three caregivers did not know the ages of the children they were responsible for. This was corrected when the director provided the children's files which included the date of birth for each child.

A caregiver who was under the age of 18 and attending high school was left alone with sole responsibility of caring for children.

The Director does not have current training in the following topics: Transportation, Health and Safety, Medications and Allergies, Child Abuse and Neglect, Building and Physical premises safety, Emergency Procedures, and Ensuring Safe Environment.

The operation employed three caregivers who were under the age of 18 and was not enrolled nor completed a childcare career program.

The operation created a high school diploma in an attempt to satisfy Minimum Standards caregiver qualification requirements. The diploma did not accurately reflect the education that the caregiver had.

The director assigned a caregiver assignments that did not match their skills, abilities, and training.

Five children and one adult reported inappropriate discipline was conducted at the operation. Three children reported a caregiver grabbed their arm forcibly. One child reported that a caregiver bent his wrist back. One parent reported that her child cries when coming to school and seems fearful. One caregiver reported that an autistic non-verbal school-aged child was confined to a dark bathroom because she was crying.

Three children reported that time-out is used when children do not sleep at naptime. Three children reported that children have to sit in a chair for the duration of naptime with no activity provided. One child reported that he hates naptime because he has to sit in a chair the whole time.

There is enough evidence to determine that one of the two caregivers who were on the field trip didn't have a camp shirt or lanyard etc. to easily identify the teacher as part of the field trip.
