I did not observe carbon monoxide detector in the daycare building.

During the inspection it was observed that the first aid kits in the classrooms were not complete and were missing items such as thermometers, scissors, and tweezers. While at the operation every first aid kit had supplies added so that they were all complete.

During the inspection it was found that there were two slides on the playground that did not have any loose fill around them. Slides cannot be placed on grass and meet state childcare safety requirements.

During the monitoring inspection it was observed that one of the classes was playing in the indoor gym rather than outside, even when the weather permitted them to be outside. This was corrected at inspection when the children moved from the gym to play outside.

Five of the fice files reviewed did not have the topics completed.

Based on information obtained during a DFPS investigation, it was determined two caregivers did not provide care consistent with a child?s habits as the same child had attempted to leave the playground the same day prior to the incident.

Two caregivers failed to intervene appropriately during a supervision incident at the facility, leading to a child exiting the premises and being returned to the operation by a bystander.

A review of the Employees Who Left Employment in 2022 Survey data from January 25, 2021, revealed that the center had not reported the number of employees who left employment in 2022 by January 15, 2023, as required.

One out of six students did not have the emergency contacts address on file.

One out of six children did not have their health statement with a signature.

Three caregivers were missing annual clock hours.

One child did not have current immunization records on file.

The operation was unable to provide a current fire inspection.
