During review multiple children's records were reviewed and found to be missing the full emergency contact information.

The operation does not have a completed sanitation insepction for the previous 2 years.

The operational policies were reviewed and found to be missing sections for the Minimum Standards online, HHSC Website, Parent participation and parental rights.

The wood chips were measured during the inspection and found to be at 4 inches. I also observed grass growing in the loose fill packing it down.

During the insepction staff trainings were observed. All records observed were missing the required training topics.

During the inspection some seedling starter planting soil was observed in a cabinet without a lock. The planting soil states to keep out of reach of children. This was corrected at inspection when the soil was removed from the classroom.

The wood chip loose fill had started to deteriorate and was not at the required 6 inches.

During the inspection children's records were reviewed. All but 1 did not have either a physician or emergency facilities information. All but 1 did not have an emergency contact designated either.

The operation did not complete the employees who left survey for 2022 by the required date.

During review of the operations fire drills only 2 out of the last 9 conducted had the children exit in the required 3 minutes.

Four personnel files were reviewed: Four of the four files were missing annual training hours. The names and amount of hours needed was provided to the director.
