A child was observed in the restroom area alone without a caregiver.

There were 37 children ages 7 to 8 years old with three caregivers making them over ratio by 2 children. This was corrected at the inspection when the group was split into two groups.

During the inspection it was observed that an individual was present at the operation without a background check.

One caregiver was observed caring for 27 children, ranging in age from 7-8 years old. They were over ratio by 1 child.

During the inspection it was observed that 8 classrooms were exceeding their total room capacity by 3 children.

During the inspection it was observed that children went to the restroom and did not wash their hands after they were finished.

During the inspection it was observed that the children now have access to a gym that was not previously inspected.

During the inspection it was observed that at least 4 of the bathroom stalls were missing toilet paper.

During the follow-up inspection five children, ages 5-11 years old, were observed using the restroom, without a caregiver nearby. These children were individually sent to the bathroom, down a hallway, while the caregiver and other students remained in their classroom.

During the follow-up inspection over half of the toilets available were without toilet paper.

During the follow-up inspection it was observed that none of the bathrooms were equipped with paper towels and all but one of the bathrooms was lacking soap.

During the follow-up inspection two children were observed not washing their hands after they had used the restroom.

During the monitoring inspection it was observed that none of the bathrooms were equipped with paper towels and all but one of the bathrooms was lacking soap.

It was observed that children were beginning to eat breakfast after they arrived, without washing their hands first.

During the inspection it was observed that the operation only had this week's daily grounds safety checklist available for review.

During the monitoring inspection it was found that a caregiver was alone with the children in care with a provisional background check that required that they could not be alone.

During the monitoring inspection it was observed that the bathrooms of the operation had strong smells of urine, there appeared to be a puddle of urine in one of the bathroom floors, and the outdoor areas had trash and debris accessible to children.

During an investigation it was found that children were being cared for without a permit.

During an investigation it was found that parents were not being permitted to enter all areas of the child care operation.
