Two diaper changing surfaces used for infants and toddlers were not non-absorbent.

A person was present and assisting a carergiver during the inspection who did not have a background check. Another caregiver was present and caring for a group of children with an inactive background check.

During the inspection the hand washing sink located near the diaper changing area in the infant classroom was not working.

This standard was evaluated during the investigation and was found to be deficient. The operation required a parent to secure prior approval in order to visit their child during the child-care center's hours of operation.

One child who has a doctor diagnosed food allergy did not have a food allergy emergency response plan on file.

During the inspection a caregiver was present and caring for children without being eligible to be present at the operation.

Four out of four staff files did not have training in storing and handling of hazardous materials, or responding to food allergies.

During the inspection one caregiver was supervising six infants under 12 months old. An additional caregiver was observed going into the classroom to assist, however the caregiver was then observed leaving the classroom multiple times during the inspection.

During the inspection a school aged child was observed coming from the outdoor playground into the front entrance of the facility unsupervised.

A 7 month old infant was asleep in an infant swing during the inspection. This was corrected when another staff member came into the room and put the infant into a crib.

The operation's last fire drill was conducted in February 2022.

The operation has a storage container accessible to children which is not equipped with ventilation holes.

One out of four child's records evaluated did not have an immunization record.

During the inspection a caregiver was present and caring for children without the operation receiving notification from the CBCU that the caregiver eligible to be present. This standard was corrected during the inspection when the caregiver was sent home.

One out of four child records was missing a physician's health statement.

The outdoor playground equipment that rocks and moves does not have loose filled surfacing the in the usezone areas. Photos were taken.

The electronic children's attendance records were not available for January to February 9, 2022.

Three infants were observed unsupervised. One young infant was alone in classroom 1 and two infants ranging from 9 months and older were alone in classroom 2. The deficiency was corrected during the inspection when the caregiver returned and combined the two infants that were in alone in classroom 2 with an infant in classroom 1.

Infants were observed in restrictive devices for more than 15 mintutes. The deficiency was corrected during the inspection when the caregivers moved the infants on the floor to engage in tummy time.

An infant did not have current feeding instrructions on file.
