One out of the four employee records monitored did not have Form 2912 affidavit.

The caregiver did not use good judgment or self-control.

Operation did not report the incident to Licensing.

Inspector observed the infant room out of ratio during the visit on multiple occasions due to not having extra staff. The director was observed leaving one caregiver with 6 infants while preparing food or conducting other duties. The infants are between 9-13 months, the specified age group is 0-11 months old.

Three out of four staff did not have the required trainings on file. One out of four staff was missing Child Maltreatment training and Emergency Preparedness training. One out of four staff was missing SIDS, Shaken Baby, and Early Brain training. One out of three staff was missing Child Maltreatment training, and Administering Medication training.

The operation was closed on November 9, 2022 due to the operation not having water. The City of Aransas Pass was working on the water lines of the operation.

There are various areas on the Infant playground that have exposed cement. Discussed with the direcctor the hazards of a child falling or tripping on the exposed cement.

The current daily menu was not visibly posted. The menu was behind the previous school weeks menu. This was corrected when brought to the directors attention.

One out of four staff did not have the required training for child maltreatment and preventing and responding to emergencies due to food or an allergic reaction.
