This standard was evaluated as part of an investigation and found to be deficient. A child was found in a classroom unattended. The child was alone for a short period of time.

This standard was evaluated as part of the investigation and was found to be deficient. Through interviews of staff, the victim, and a collateral adult, and through review of video, it was determined a caregiver was not properly supervising the children in care when one young child left the classroom without their knowledge and exited the building. The young child was returned to the operation by an adult not associated with the operation. The young child was unsupervised by the operation for 5 minutes and 46 seconds.

This standard was evaluated as part of the investigation and was found to be deficient. The parent of a young child who was unsupervised was not immediately notified of the incident. The parent was notified one hour and 30 minutes after the incident occurred.

Children were observed napping on mats not labeled with a child's name or with a number and have a map listing the name of the child that is associate with the number. This will help reduce the spread of germ. The caregivers sanitized the mats after nap time, and they will label the mats moving forward.

The operation did not provide documentation to validate that a current fire inspection was conducted by a local or state fire marshal.

Written feeding instructions for three infants who do not eat table food were not updated within the past 30 days.
