The toilets in the purple, orange, and green rooms were observed to be dirty and in need of maintenance. The garbage cans in the restrooms were observed to not have a lid making the garbage accessible to children.

A caregiver was observed to be supervising children alone without a pediatric first aid with rescue breathing training.

Three out of ten employee records evaluated did not have the following trainings: hazardous materials, allergic reactions, administering medication, emergency preparedness, building safety, and communicable diseases.

There is one sink in the infants' room. Sink is being used for purposes other than washing hands.

Surfacing on use zone of two slides over 20 inches tall located in the outdoor playground was under 2 inches in depth.

Sweet tea with added sugars was served during lunch on this day as per caregivers, a child, and observations made.
