The use zone in front of the to/fro swing set does not meet the footage needed - which is 11 feet, based on the vertical distance.

The following items were observed on the sides of the shed in the backyard play area: stacked boxes, bricks, and debris.

Admission forms were not available for 2 of the children in care today.

I observed the new ac unit was installed, but the safety fencing was not around the new unit.t. The old unit was still in the yard also without any fencing around it.

The outdoor play space had bricks and other hazards on the ground behind a shed that needed to be removed. There was a cracked slide attached on a play structure that presented a pinching hazard. The corner of the outdoor play space had bricks stacked up that presented a hazard.

The primary caregiver was missing one required training: Preventing and controlling the spread of communicable diseases, including immunizations

The operation did not have a carbon monoxide detection system on the second level.

The air conditioner unit located outside of the home in the backyard was accessible to children.
