One infant was in a crib for naptime with a bib on and there was a changing mat on top of the play yard. This was brought to the provider's attention and she removed the bib and the changing mat during inspection.

The provider did not have a written activity plan.

A household member who resides at the home did not have an eligible background check. The household member left during inspection.

Operation does not have liability coverage as of 2/24/23.
