The person in charge left the school age teacher with 40 children. The second teacher that was in the classroom left early. The person in charge was assigned to help the school age teacher but she left the classroom to help the pre-k teacher. The director was not present at the daycare when this occurred.

A caregiver was alone with 40 school age children. There were seven children that were 5 years old and thirty three children 6 years and older. THe caregiver was alone for 4 minutes while the person in charge assisted the pre-k teacher.

During the course of a DFPS investigation it was determined that the director acknowledged and admitted being out of ratio for the last two weeks with 28 school age children to one caregiver.

During the course of a DFPS investigation it was determined that a caregiver failed to apply good judgement when the incident occurred. The caregiver failed to physically intervene and separate the children while they were having a physical altercation. The caregiver was sitting on the playground steps and was tying other children shoelaces.

During the course of a DFPS investigation it was determined that a caregiver was out of ratio when the incident occurred. Caregiver had 28 children in the school age group. Staff admitted being out ratio for the last two weeks with 28 children to one caregiver.

There was no documentation that a gas inspection has been conducted at the center this year. The last gas inspection was on 12/28/2018. The operation must schedule an appointment for an inspection.
