One out of the ten staff records reviewed during inspection did not contain a current First Aid and CPR training.

While reviewing child records, 4 of 6 did not have a health care statement included in file.

While reviewing staff files, one staff had 13 of 24 required hours completed; another had 5 of 24 required hours completed.

Based on the finding a four-year-old child was disciplined by being restrained and put into a highchair for as long as forty-five minutes for the purpose of behavior.

Based on the finding a four-year-old child was physically restrained by staff while he was angry and uncooperative. He was further reported to have been dragged by his hands to be placed in a highchair as a form of discipline.

Based on the findings a four-year-old child was placed in a highchair that is designed for the purpose of feeding infants who are unable to sit on their own. The child may have exceeded specified weight and height specifications of the of the manufactured product as it was designed primarily for use with infants and toddlers, not preschool aged children.
