At the time of inspection the operation's last gas leak test was performed on 4/14/21.

At the time of inspection several staff missing annual safety trainings in files.

Fire drills have not been practiced or documented.

At the time of inspection the opertion did not have a current CPSC paperowork on file.

At the time of inspection the operation did not have a current health and sanitation inspection.

At the time of the inspection an epi pen was stored at the operation for a child with a food allergy, but the epi pen was not in the prescription box container with the child's name one it.

At the time of the inspection the operation had not had a fire inspection in the last 18 months. Exceeding the requirement to have a fire inspection every 12 months.

It was found through investigation that a concern was raised about an employee and the director did not ensure the concerns were addressed in a way that resolved the issue.

It was found through the investigation that a caregiver was too strict for the ages of children they were providing care for. The caregiver was found to have had a stern tone when giving instructions and managed behaviors with frequent time outs. The caregiver's temperament was not appropriate for the group of children she was supervising.

It was found that a concern had been raised about a caregiver's interactions with the children regarding their tone with the children and length of time outs in the classroom and no report was made to Childcare Regulation.

It was found through investigation that a caregiver would abruptly take food from children if they felt the child was not eating in an appropriate manner. An example would be not using their utensils to put food in their mouth, or taking food apart to eat it.

At the time of the inspection written feeding instructions were not updated for 7 infants within the last 30 days.

At the time of the inspection multiple bottles stored in the infant fridge did not have names labeled on them.

Based on information obtained during the investigation it was found that the operation was aware of an injury that required medical attention and did not make the required report to Childcare Regulation of the incident.

Based on information obtained bolts on the fencing protruded in a way that led to a child being injured.

Based on information obtained through investigation it was determined caregivers did not maintain supervision over a child when the child was able to exit the playground and operation property.

Based on information obtained through investigation a caregiver did not exhibit competence in her role when a child was unaccounted for and missing from the group without her knowledge.

It was found through investigation that a caregiver was left on the playground area with 12 children, 2 years of age, exceeding the ratio requirements for 1 caregiver present.

At the time of the inspection the older infant classroom exceeded the maximum group size by 1 child during the visit with 2 caregivers in the classroom. This was corrected when 2 children from the classroom were observed to be picked up from care for the day.

At the time of the inspection one infant, over 12 months of age, was observed seated in a high chair without the safety straps fastened. This was corrected when staff were spoken to and removed the child from the chair.

At the time of the inspection a cabinet was observed in a classroom that was not secured to the wall and was observably shaking at the touch. The shelf poses a tippiing hazard.
