A Review of the Employees Who Left Employment in 2025 data from January 21, 2025, revealed that the center had not reported the number of employees who left employment in 2025 by January 15, 2026, as required.

The Governing Body, Anchor of Hope Apostolic and Prophetic Ministries, Inc. is still showing as forfeited, making the operations Permit to operate in-valid. The operation must provide documentation to support the governing body as current as soon as possible, but no later than one week.

The following safety hazards were observed: . The shelf in the infant room was not anchored to the wall. This shelf is light and could be easily pulled over. The table where the small refrigerator is placed in the infant room is light and could be pulled over. A different type of table or shelf should be used. The cord to the refrigerator was accessible. Accessible extension cords were observed in the school age room/cafeteria area. A cord was accessible in the three year old room but a shelf was moved to make it inaccessible. Several of the fence boards on the playground were observed to be loose or missing.

Two of the doors used as fire exits were locked and not easily opened from the inside.
