The back yard fence had multiple board that need to be replaced. The stairs railing is loose.

A caregiver did not adhere to the supervision requirements outlined in the child's service plan by leaving a child on 1:1 supervision unattended for approximately 40 minutes.

Two caregivers failed to maintain self-control and engaged in inappropriate behavior towards each other in front of the children in their care.

During the course of the investigation, documentary and testimonial evidence indicates that the principals and collateral witnesses were briefed and prepared to respond to interview questions.

The operation placed a child who was younger than the victim in the same room, against the victim's Safety Plan which clearly noted the victim should not share a room with any child younger than them.

Inspector reviewed morning and evening medications logs for all children in care, randomly selected medication to count, and reviewed the training roster and medication administration-documentation training certificates. Three medication logs were reflecting a discrepancy with the remaining medication count by 1-2 pills. Inspector conversed with the permit holder regarding the operation's plan to ensure compliane with medication documentation.

There was no documentation available for review related to the post-discussion required with child.

All medication logs were evaluated, and some were observed to be missing the information required to identify the staff who administered the medication.

Staff member pre-documented the administration of evening medication, which resulted in discrepancies in the daily medication count on multiple medication records.

Several incident reports were observed to be incomplete and/or missing required information.

A restraint report dated February 22, 2025 at 11:15 AM was not reviewed within the 72-hour timeframe which should've been by February 25, 2025 at 11:15 AM. The supervisor date and time was blank.

All medication logs had an inaccurate daily count due to incorrect subtraction from the starting count.

Medication logs include the signature and a cell for staff initials. The documentation doesn't include the name of the person who administered the medication, and several dates doesn't have an initial to match the person who administered the medication.

Employee files contain the names of the references and indicate that the individuals were contacted, but the pre-employment screening doesn't include the documentation of the conversations.

The second level of the home was observed with wasps nests outside of the home.
