This standard was evaluated and found to be out of compliance when a caregiver failed to notify a child's parent at pick up time, of red spots on the child's body that the medical practitioner diagnosed to be cellulitis.

One child's file, aged five years did not have a current immunization record. The child is missing 1 Dtap, 1 Hep A, 2 Hib, , 1 MMR. 1 Polio, and 1 varicella.

One child did not have current health statement information on file.

Five children did not have current immunizations on file at the operation. Two children were missing the 2nd and 3rd does of immunizations for 2-6 months. One child was missing the 2nd Hep A. One 2.5 year old child has record of first 0-2 month immunizations only. One child did not have any immunizations on file.

Staff do not have current training on handling/ disposing of hazardous materials, medication administration, or handling food allergy emergencies.

This standard was evaluated and found to be out of compliance when a caregiver did not use positive methods of discipline and guidance appropriate for the child's level of understanding during a behavior redirection.

This standard was evaluated and found to be out of compliance when a caregiver handled a child harshly redirection.

Children's hands were not washed after diapering.

A caregiver changing diapers did not change gloves or wash her hands in between diapering children.

A toddler classroom of mixed ages 15 months - 24 months was out of ratios by four children. There were six children under 18 months and four children 19 months old. The specified age for this room was 12 - 17 months. This was corrected at inspection when an second staff was added to the classroom.

Child Care Center Director and Second Director have not completed their 30 clock hours of annual training.
