A review of the Employees Who Left Employment in 2024 Survey data from January 17, 2025, revealed that the center had not reported the number of employees who left employment in 2024 by January 15, 2025, as required.

The infant room had 2 caregivers caring for 12 infants. This classrrom was out of ratio based on the specified age group of 0-11 months. The classroom was over ratio by 2 infants.

The infant room did not have a sink next to the diaper changing table.

The diaper changing table was observed to be not clean. The diaper changing table was observed with white and brown dirt stains on the surface.

Two vans did not have a working vehicle safety alarm.

After reviewing staff files there were six employees who has taken CPR/First Aid online
