The school-age children attending before school were being served cereal regularly instead of what was on the daily menu without substitutions being recorded.

Due to recent heavy rain there is an area of soil that has been depleted on the far corner causing a drop from the leveled ground located near the pre-school/after-school playground. (Note: children are not allowed in this area of the outdoor play area.)

Two chlldren have a known food allergy although there is no food allergy plan on file.

There were small screws and brackets on top of the 18 month- 3 year old playground plat form. Corrected at time of inspection whent the director removed the screws and brackets from the playground platform. There are several high weeds in the prospective toddler classroom.

The prospective pre-school/school age playground fence has a 6 inch gap.
