During the course of the investigation, it was determined that a caregiver used prohibited punishment with a child in care.

During the course of the investigation, it was determined that the report to Child Care Regulation was not made within the required timeframe after operation staff became aware of the incident that placed a child at risk.

During the course of the investigation, it was determined that a caregiver did not demonstrate good judgment and self-control during an interaction with a child.

Four caregiver files were reviewed found to be incomplete. Each file did not meet the requirement of having completed a minimum of 15 clock hours of annual training. The training hours were incomplete for the following required topics: 747.1309(b)(1) minimum of six clock hours in child growth and development, (2) guidance and discipline, (3) age-appropriate curriculum, and (4)teacher-child interactions; (c)(1-4) at least one clock hour that focuses on prevention, recognition, and reporting of child maltreatment; (d)(1) emergency preparedness, (2) preventing and controlling the spread of communicable diseases, (3) administering medication, (4) preventing and responding to food allergy emergencies and allergic reactions, (5) understanding building and premises safety, and (6) handling, storing, and disposing of hazardous materials.

The caregiver who was left in charge in the site director's absence was not familiar with this site's emergency procedures.

Documentation revealed that the incident report to the parent was not given within the 48 hours of when the incident occurred.

Protocol was not appropriately followed and a child was left on the playground for a few minutes without supervision.
