A caregiver did not use good judgment when they were playing with children in care by pushing them very high in swings constructed of hard plastic designed for physically impaired children, resulting in a child sustaining an injury that required medical attention.

Licensing was not notified in the time required of a change in site directors.

One child with a food allergy did not have a Food Allergy Emergency Plan in their file at the time of inspection.

The site director was unable to pull up the admission records on the electronic tablets that were at the operation. As a result, ten children records were not available to be evaluated during the inspection.
