During the inspection two children were observed coming from the gym to the cafeteria with no supervision. The caregiver in the cafeteria was no aware of the children's present and the caregiver responsible for the children was observed still inside the gym.

The operation does not have an accommodation plan for a child in care who is enrolled with an identified special need.

The caregiver supervising the child at the time of injury does not have a first aid card, and CPR is only online.

The site director has 6.5 out of 15 hours required for the 8/2023-8/2024 training year. The site director does not have any of the require topics in health and safety.

The operation's site director CPR/First Aid is expired. One zone leader that has been employed for over 90 days have online only CPR.

Based on information gathered during the investigation, a caregiver did not intervene to assist with toileting while the child stood with fecal matter on his body and was wet for 20 minutes.
