One child was observed throwing a football inside the building onto the stage of the cafetorium striking another child in the head.

The operation has not notified Licensing about the change in the program director. There has not been a qualified program director for the operation since 11/7/2022.

The operation did not have sign in and out sheets at the time of inspection.

One child's allergy medication was not in the original container and contained no label. This deficiency was corrected at inspection when the site director discarded the medication.
