Two children in care with special needs did not have accommodations recommended by a health care professional or a qualified professional affiliated with the local school district.

Based on the information gathered during the investigation, it has been determined that child was left unattended, and caregiver was not at a physical proximity which did not allow visual and/or auditory awareness of the child in care.

Based on the information gathered during the investigation, it has been determined that the school failed to report to Child Care Regulations that a child had been involved in any on-routine situation that placed, or may have placed, the child at risk for injury or harm when left unsupervised for a period of time within the required timeframe.

One child was using their chromebook during snack time to look for the theme song to the movie Halloween. Two children were using their chromebooks during snack time.

Two caregivers did not have required annual training.
