The operation failed to report a director change occurred. It was reported the site director left on 12/20/24 but controlling person record states site director was reported end date was on 12/2/24. The operation just reported this change on 1/10/25. This is past the 5 days required by minimum standards.

A caregiver who is caring for children unsupervised does not have required Pediatric First Aid training.

The site director did not have 3.25 hours of required annual training.

The site director left children alone and unsupervised in the gynasimum when they exited the gym to answer the door. One caregiver was not aware that a child was about leave the area unescorted.

One group was observed not washing hands after outdoor activity.

The Site Director's Licensing Director's Certificate was not in the operation's binder.

The inspection form from the most recent child care regulation inspection was not in the operation's binder.

Children were served Capri Sun Fruit Punch during snack time.

Food allergy emergency forms for two children with food allergies were not available for review during the inspection.

This standard was evaluated during the investigation and was found to be deficient. The operation did not communicate with the parent immediately after the child sustained and injury which required medical treatment.
