The center did not follow their own emergency plan after they transported a young child to a school she was not enrolled in error and left her there in an unfamiliar place.

A 3-year-old child was transported and left at a school the child does not attend in error and Child Care Regulation was not notified of the incident.

An incident report was not completed and signed by a parent after a child was transported to an unauthorized location and left there.

Based on the evidence gathered, a 3-year-old child was dropped off at another daycare in error and the parents were never notified of the situation.

The parent of a child in care was not given the opportunity to review the new admission form the staff had completed on the child and make any needed corrections. As a result, the child was transported to a school in error and left there based on inaccurate information on the new form. This would have been prevented if the parent had been able to review the new admission form.

Based on the video footage observed, two children were observed fighting while the caregiver was standing in the doorway. The children were observed fighting for 1-2 minutes without proper supervision. The caregiver's back was to the children in care, which resulted in a child being scratched.

Eight out of ten staff records acquired online CPR/First-Aid certification. Three staff CPR/First-Aid certification was dated 08/10/2020 and expired in 08/10/2022. The Director scheduled an appointment for 11/2/2022 afternoon while Licensing was present.

Staff did not show good judgment when moving a toddler feeding table, with 8 toddlers sitting inside of it. Table was moved for cleaning purposes and collapsed injuring a child.

The pre-k classroom and the preschool classroom both had the television on during naptime. Note- This was corrected during the inspection when the televisions were turned off.

Multiple infant bottles were not properly labeled with the infant's first name and last initial. Note- This was corrected during the inspection when the infant bottles were properly labeled.

A piece of climbing equipment was placed on grass. Note- This was corrected during the inspection when the play structure was moved onto loose-fill.

A two-month-old, a six-month-old, and a 10-month-old were asleep in their cribs with bibs around their necks. Note- This was corrected during the inspection when the bibs were removed from the infant's necks.

Three out of four caregivers who have been employed for more than a year did not have annual training in the required topics of Handling, Storing, and Disposing of Hazardous Materials, Administering Medications, Preventing and Responding to Emergencies due to Food Allergies, Building and Premises Safety, Preventing the Spread of Communicable Diseases, and Emergency Preparedness.

A piece of stationary equipment which included a small slide did not have the required six feet of use zones maintained. Note- This was corrected during the inspection when the area was cleared out to allow for six feet of use zones around the play structure.
