A caregiver was observed being counted in ratio in the infant room without completing pre-service training.

Two different infants were observed sleeping in a restrictive device on two separate occasions for over 5 minutes each. *This was corrected at inspection when each child was removed from restrictive device.

The operation failed to notify parents of safe sleep deficiency within the required time frame.

A caregiver was observed feeding an infant. Caregivers eyes were observed closed and resting her head on her left shoulder.

Recite: A caregiver hired on 11/18/22 completed only 19 out 24 hours of Pre-service trainings.

The ceiling panel for one classroom was not replaced and continues with water stains. The other classrooms were observed with clean ceiling panels and the classroom previously with a loose ceiling panel was fixed.

There were three classrooms used by children with dark brown water stains on the ceiling panels. There was one classroom with a loose ceiling panel. The vents in the operation were observed clean and properly installed.

A caregiver hired on 11/18/22 completed only 14 out 24 hours of Pre-service trainings.

There was no activity plan created for the month of May 2023 in the infant classroom. The last activity plan posted was for April 2023.

One classroom with children under the age of 5 years old did not have covers for 2 outlets. Corrected at Inspection: The covers were placed on the outlets. One classroom did not have a wall plate cover for an outlet.

An infant was asleep in a crib with no sheet covering the mattress.

Training records were not available for review.

There were four classrooms with a vent full of dust and debris. Two of the vents were loose from the ceiling which pose a risk to the children if it falls.

The CPR training was not current for the director.

Based on the information gathered during the investigation, the operation staff failed to follow the child-care center policies by releasing a child to an authorized person without verifying their identity.

Five caregivers did not have the required 24 hours of Pre-Service Training.

The Food Allergy Plan for a child diagnosed with a food allergy was not posted in the classroom. Corrected at Inspection: The Food Allergy Plan was posted in the classroom.

There were several vents observed in the classrooms with accumulated dust and debris.

The front play area was observed with a broken tile and a torn section of the unitary material which pose a tripping hazard for the children during outdoor play. Red ants were observed on the play area that was being used by the children. Two classrooms were observed with accumulated dust and debris on the vents.

A CPR was done online by a caregiver. Another caregiver did not have a CPR training in pediatrics.

The health inspection report was not available for review.

A caregiver allowed several children to climb up the slide several times putting the children at risk of falling. The caregiver did not acknowledge the information provided to her regarding the risk of the children falling.

The Infant Daily Report was missing the section: Summary of Activities.

The time sheet for the Director was not available for review.

The Food Allergy Plan for a child diagnosed with a food allergy was not posted in the classroom and kitchen. Employees were unaware of the child's allergy symptoms and treatment. Corrected at Inspection: A copy of the Food Allergy Plan was posted in the operation.
