Observed a 2 month old child asleep in a bouncer that was placed inside play yard while child was covered with a blanket.Note: Child was removed from bouncer and blanket was removed.

Facility has not received background check results for an individual that was present at the operation in a classroom supervising 3 children. A second person supervising a group of 6 children does not have a current background check. A third individual was present and facility has not submitted a background check.

Facility is moving to a new location and Child Care Regulation Department has not been notified.

Vehicle that facility is using for transportation is missing the following: A list of the children being transported; Emergency medical transport and treatment authorization forms for each child being transported; Parent's names and telephone numbers and emergency telephone numbers for each child being transported; A fire extinguisher approved by the local or state fire marshal, secured in the passenger compartment and accessible to the adult occupants and a first aid kit as specified in 746.4003 of this title related to What items must each first aid kit contain.

Employee's attendance is not current for director. Therefore, unable to evaluate hours present at operation.

Based on the information found during the investigation the center failed to report once the found out that a child had received medical attention.

In the infant classroom there was a baby walker with wheels which is considered prohibited equipment. NOTE: This was corrected at inspection when caregiver removed the walker out of the classroom.
