During the inspection, one out of the eight children's files was missing contact information for the authorized pick up persons.

During the inspection, the director's file was missing annual training hours in required topics.

During the inspection, four out of the seven staff files reviewed were missing training hours from the last full year of employment.

During the inspection, seven out of the eight staff did not have Center Policy acknowledgments available for review.

During the inspection, there were two caregivers present for the two classrooms. One of the caregivers had to leave their classroom and children unattended to unlock the front door.

During the inspection, the infants were left unsupervised when the caregiver left the classroom to answer the front door, then proceeded to another classroom to talk with another caregiver.

During the investigation, it was found that caregivers utilize a highchair to restrain children in care.

A caregiver stated that she had eleven children in care. She actually had twelve in care.

During the inspection, it was observed that two trashcans accessible to children in restrooms did not have lids.

During the playground inspection, a slide was observed on the asphalt playground, with no loose fill material.

During the playground inspections, the chain link fence was found to have protruding pieces on the bottom, causing a safety hazard. There was a battery found in a playhouse. A tarp was on the ground of the playground, posing a potential suffocation or entanglement risk. The bushes from the neighboring house are overgrown through the fence into the playground. These plants have sharp, thorny leaves. There are places on both playgrounds where the weeds are overgrown.

One out of four children's files did not have up to date immunization records.

The changing table pad in the infant room had a cloth covering, which was not able to sanitize in between diaper changes. NOTE: This was corrected at inspection when the cloth cover was removed.

Floor cleaner was observed accessible to children as it was located in an unlocked cabinet under the sink in the children restroom. Note: Corrected at inspection when the cleaner was moved and placed on a high shelf out of reach.

The operation has not reported the number of employees that left their employment in 2023

A child's food allergy was not observed posted in the correct classroom. Note: This was corrected at inspection as the food allergy form was posted in the correct classroom during inspection.

None of the children napping were on the correct designated cots. Cots were labeled with children's names during this inspection.

Operation fire marshal inspection was last conducted on 11/17/2020.

Three small flat screen televisions accessible to children were not properly anchored. Corrected during inspection, television were removed and placed in storage.

The daily reports for two infants observed in care were not being created and shared with parents. Note: This was corrected at inspection as the caregiver began documenting the daily reports for the two infants.

One out eight children's record reviewed during the inspection did have an immunization record for review. Two out of the eight children's records reviewed did not include the required health statement.

The emergency preparedness plan reviewed during inspection did not include all the required topics.
