Alice's Daycare

Two of four files reviewed were incomplete. The files were missing a copy of the high school diploma. The employees were hired after 2023. They were also missing the pre-employment affidavit.

A review of the Employees Who Left Employment in 2023 data from January 22, 2024, revealed the center had not reported the number of employees who left employment in 2023 by January 15, 2023, as required.

There were no training hours available for review at the time of inspection.

Children's Products annual certification was last completed on 6/27/22. Completion of certification is overdue by 5 months.

3 fire extinguishers have not been serviced. Last serviced 9/2022.

Annual fire inspection has not been updated. Last inspected on 9/26/2022.

Out of the staff files reviewed, Caregiver had no verification of training hours.

The sign in and out log was reviewed. There were thirteen children present and none signed in. Last sign in log was 5/2023 at the time of inspection.

Observed one staff and one caregiver in charge of a group of children present in the operation without having had a background check run by the operation.

There is no surfacing under the seesaw in the playground. The seesaw was removed during the inspection.

The transportation binder did not include the emergency medical forms for the children being transported. These were added during the inspection.

The Children's Product Certification was last completed on 3/11/2020.

One of Four files reviewed was missing: the date of admission, special care statement, food allergy question, health provider statement, and health care professional orders.

Training records were not available for review.

Observed one school age child watching cartoons on the ipad at time of inspection. Program on ipad was not related to set activities. Another school age child was observed to be watching videos and playing games on his mobile device. NOTE: This was corrected at inspection as mobile device and ipad were put and turned off. Children were then provided with an appropriate activity.

Observed 3 children doing virtual learning exceeding the two hours of screen time. NOTE: This was corrected at inspection as waiver/variance was requested at the time of inspection.

As of November 2020 caregiver is no longer associated with the operation at the time of inspection and had not been inactivated. NOTE: This was corrected at inspection as caregiver was inactivated at the time of inspection.

Observed 3 children between 13 months old to 3 years old watching cartoons on the television at the time of inspection. Note: Television was turned off and removed from the classroom.
