Current activity plan is missing the following: -List the dates (daily, weekly, or monthly) the plan covers -Afternoon opportunity for outdoor play -Appropriate / allowable screen time usage

Backyard has overgrown weeds and construction material. There are 7-8 inches wide gaps between fence and wall.

Provider has not been taking the following information during the last transportation routes: -List of the children being transported -Emergency medical transport and treatment authorization and Parent's names and telephone numbers and emergency telephone numbers for each child being transported Note: Photographs of the children's admission forms were taken with cellphone provider takes during routes which include Emergency medical transport and treatment authorization and Parent's names and telephone numbers and emergency telephone numbers for each child.

Provider could not locate transportation training at the time of inspection.

Operational policy on animals was not available for review. Operation has a dog in the premises.

Immunization records for two children were not available for review during inspection.
