One is missing copy of high school diploma / GED. Three are missing certificates for trainings on communicable diseases, hazardous materials, food allergies and emergency preparedness.

Six children's files were reviewed. One file is missing immunization records and six files are missing parent's rights statement.

Last fire drill was conducted on 05/2023. One severe weather and one lock down drills were conducted during the last calendar year. Last smoke detector, CO detector and fire extinguisher checks were documented on 01/2023. Note: provider completed a CO detector and fire extinguisher check during inspection.

A caregiver was present and caring for children prior to operation receiving background check eligibility notice. Note: This was corrected at inspection when caregiver left operation.

Operation has a caterpillar crawl and climb surrounded by 2-6 inches of tire mulch. There is also a play structure with two slides and a height of 24 inches without surfacing.
