During the inspection, 1 staff member did not have the pre-service mandatory reporting training on file and has been employed as of October 2024.

During the inspection, there was not a staff member on site who could provide records for immediate review. Licensing representatives waited approximately 45 minutes for a staff member to arrive on site for assistance. Licensing representatives then had to wait an additional 45 minutes for records to start being provided, upon the staff member arriving to the operation and having a scheduled interview with a potential employee. There was not another staff member who could provide records.

During the inspection, 4 employee files did not have documentation of verification of employment made by the operation. Some documentation regarding verification of employment was incomplete or there was some partial information listed.

The operation has children in care but hasn't conducted a severe weather drill.

In a review of documentation provided, one child in care's serious incident report did not have documentation of the parent/managing conservator being notified, along with no date/time for the notification. Additionally, the incident report did not have documentation of the date/time for the notification to law enforcement.

In a review of documentation provided, one child in care's initial admission assessment did not have documentation of her medications. It was documented that the child is diagnosed with a medical health condition and would take her medications as prescribed, but her medications were not documented on the assessment.
