The fire extinguisher in the operation's van needed to be serviced as the needle was in the red. This was corrected during the inspection when an extra fire extinguisher from the center that was appropriately pressurized was placed inside the van and the original fire extinguisher was removed from the van.

The first aid kit in the van was missing a first aid guide. This was corrected during the inspection when a first aid guide was printed out and added to the van's first aid kit.

Out of six children's files reviewed, one child was missing school information and another child needed an updated immunization record.

The loose-fill at the slide exit had been displaced and was not at the required depth.

Three rooms did not have an evacuation diagram posted in them. This was corrected when extra diagrams were printed and posted in the rooms.

There was a garbage can without a lid on it that had diapers and food inside. This was corrected during the inspection when the garbage can was removed from the room and replaced with a garbage can that had a lid.

The parent handbook was missing procedures for water activities, information on the benefits of physical activty, criteria for when bad weather prohibits outdoor play, a plan for physical activity to occur indoors when children can't go outside, and Child Care Regulation contact information. This was corrected during the inspection when this information was added to the handbook.

There was a bin of cleaning supplies on a low sink within the reach of children. This was corrected during the inspection when the caregiver moved the supplies to a cabinet out of the reach of children.

There was one volunteer who was regularly present and did not have a background check run at the operation. This was corrected during the inspection when the volunteer left the operation.

The loose-fill underneath a merry-go-round and a climbing structure was not six inches deep as required.

There were several structures where the loose-fill had been pushed out of place and was not at the proper depth or had been entirely displaced.

There were several live anthills on the playground.

One uncovered electrical outlet was observed in a classroom of 2 and 3 years olds. The outlet was covered during the inspection.

It was observed that the loose fill below one outdoor play structure was not being maintained at the required depth. The tarp lining below the loose fill material was observed to be exposed, posing a tripping hazard.

There was sufficient evidence found during the investigation to show that a child was left unattended in the operation's transportation vehicle.

There was sufficient evidence found during the investigation to show that a 4-year-old child was neglected when they were left in the operation's transportation vehicle before exiting the vehicle and crossing a busy roadway in the area, unattended.

It was found during the investigation that one caregiver did not comfort a child in care for several minutes when the child was crying in a classroom.

It was found during the investigation that one caregiver yelled at and used a harsh, negative tone with toddler aged children in one classroom.

One playground slide structure and a swing set observed did not have appropriate active play surfacing in the use zones.

Four infants were observed eating at a feeding table. The seat straps were not fastened while the infants were using the equipment.

One prescription and one over the counter medication were observed in the infant classroom. There were no records of medication administration to review.

One caregiver was observed to be solely responsible for 11 two year old children and not in compliance with the conditions of a provisional background check. The caregiver was transferred to another classroom accompanying an eligible caregiver.
