In two of the classrooms, the ceiling fans had dust collected on them. This was corrected at inspection when the director cleaned the ceiling fans.

Based on the information provided, an incident was not reported to CCR within the 2-day timeframe.

Children ages of 6 years old to 12 years old feeds, give water, and gather eggs from the chicken coop. This was corrected at inspection when the operation agrees to no longer allow the children to have access to the chickens.

During the inspection it was observed that the toddler's playground had several spiders and spider webs on their outdoor playhouse. This was corrected at inspection when they were all removed.

During the inspection it was observed that a caregiver wiped a child's nose and then wiped another child's nose before washing their hands or using hand sanitizer. This was corrected at inspection when hand sanitizer was used between wiping noses moving forward.

During the inspection it was observed that there was an individual present at the operation with direct access to the children before the operation had received notification from the CBCU that the subject of a background check was eligible to be present. Volunteer left operation while Child Care Regulation was present.

This standard was found deficient as part of a DFPS Investigation.

The operation failed to notify parents of a child in care that was placed outside, on the porch without adult supervision.

One caregiver whom has been employed for at least 90 days has CPR training from an online resource instead of a hands on skills component.

A child in care was sent to the backporch of the of the toddler building after starting to cry during nap time. The child is care was sitting on the ground upset. The door to the backporch was closed and both staff members were in the classroom. Neither staff member had auditory or visual awareness of the child placed on the porch.

A fabric floor rug was used for diaper changes instead of a waterproof non-absorbent surface. This also prevented the diaper changing surface from being sanitized after each diaper change.

Three caregivers are lacking a notarized affidavit of employment in their file. Two caregivers also were lacking documented orientation with each topic selected with a signature.

A caregiver was conducting diaper changes and was wearing gloves. She did not remove the gloves at any point in between diaper changes and did not wash her hands in between diaper changes.

Four caregivers, including the director's training record, was lacking several health and safety audit including: preventing and responding to emergencies due to food or an allergic reaction , understanding building and physical premises safety, including identification and protection from hazards that can cause bodily injury such as electrical hazards, bodies of water, and vehicular traffic; and handling, storing, and disposing of hazardous materials.

The operation did not have a copy of the most recent sanitation inspection report available for review. The last sanitation inspection was for 10/26/2020.

The operation did not have the appropriate Food Allergy Emergency Plan for One child with a diagnosed food allergy. There were no indications that the instruction form present was provided by a medical professional as there was no physician signature on the form.

The operation did not have the most recent Licensing Inspection Report-Application posted during the inspection. *This was corrected at inspection when the most recent report was posted.

Three staff have been present at the operation without cleared background check eligibility. The background checks were submitted on 8/9/2021, and are still pending as of 10/6/2021.
