During the inspection, 3 children with documented allergies, did not have an allergy plan signed by the parent or doctor. This will be cited as deficient.

During the inspection, a caregiver was missing 6 hours of the required annual trainings.

The report to Child Care Regulation was made 6 days after the incident.

During the monitoring inspection it was found that the annual fire inspection was past due, with the last inspection completed May 2021.

A review of the Employees Who Left Employment in 2021 data from February 23, 2022, revealed that the center had not reported the number of employees who left employment in 2021 by February 22, 2022, as required.

During the inspection 5 out of the 10 children records reviewed were missing the following items: 3 out of the 10 children records reviewed were missing required health admission 2 out of 10 children records reviewed were missing signature regarding special needs.

During the inspection 3 out of the 11 children records reviewed did not have a health statement or admission health status completed.

During the inspection it was found that 8 out of the 11 children records reviewed did not have the admission date recorded.

During the inspection it was found that two classrooms were utilizing screent time activities for children under the age of two. This was corrected at inspection when the television was turned off in both classrooms.

During the inspection it was observed that the operation did not have a posted lesson plan in the classrooms.
