During review of the video for the investigation, it was observed that 3 infants were grouped with children who were aged 18 months or older than the infants; there was a total of 15 children in the group.

The operation did not provide the parent with an incident report after their child was placed at risk while in care.

The operation did not report an incident that placed children at risk to Child Care Regulation within the required time frames. The incident occurred on 2/23/2024 and was not reported to Child Care Regulation until 2/26/2024.

This standard was evaluated as part of the investigation and was found to be deficient. The director did not ensure that the operation was following minimum standards when the director did not ensure a caregiver was properly trained in the operation's policies and procedures before placing a caregiver in a room to work with children. In addition, the director did not ensure reports were made to parents and to Child Care Regulation within the required time frames.

This standard was evaluated as part of a DFPS investigation and was found to be deficient. When law enforcement informed the operation of abuse and neglect allegations, the operation did not make a self-report within 48 hours.

During the course of the DFPS investigation, it was determined that there was not a caregiver present on the second level of the soft contained play equipment when children were playing on the second level of the play equipment.

During interviews during the investigation inspection, it was determined a caregiver with a provisional status for their background check was left alone with a group of children in care on 7/12/2022. During the investigation inspection on 7/19/2022, the same caregiver was observed alone with a group of children in care. This was corrected at inspection on 7/19/2022 when another caregiver joined that caregiver with the group of children.
