A serious incident occurred concerning a child in care, and the parent of the child in the incident was not notified.

The outdoor surfacing has fastening bolts the are raised above the ground level creating a tripping hazard.

Two of the 10 staff records reviewed did not have CPR training that provided practice on a manikin.

A child that has been enrolled for more than 12 months does not have a health statement on file from the health care professional.

The food allergy plan for a child with doctor diagnosed food allergies did not include what foods the child is allergic to and possible symptoms if the child is exposed to said food.

For the 2022 calendar year, March, April, May, and June fire drills were not documented.

Of the five random student files reviewed, one did not have immunization information.

The walls in the indoor gym had damage to the bottom of the wall. Holes in the wall were about three feet by 6-8 inches.
