The required annual training on the following topics had not been completed by caregivers. The required annual training topics are 1. emergency preparedness 2. communicable diseases to include immunizations 3. administering medications 4. preventing and responding to emergencies due to food or an allergic reaction 5. building and physical premises safety and 6. handling, storing, and disposing of hazardous materials.

The operation did not submit the number of employees who left employment by the due date and extended due date due to portal issues.

The annual fire inspection was due by 07/20/22. The inspection had not been conducted.

A volunteer was seen to function in a caregiver role. The volunteer was unsupervised when she took children to the restroom and was in charge of the toddler classroom. The volunteer requires a background check to be submitted with FBI fingerprints.

Three children's records did not have the updated immunization record on file.

The children's records reviewed did not have the date of admission on it.

The results for a volunteer, who was present in direct care of the children, had not been received.

The operational policies were incomplete and/or lacked all the required information, reference standard 746.501. The operational policies lacked 746.501(8, 9, 11, 12, 13, 19, 21, 22, 23, 24, 25, 26, 28, b 1-2).

The first aid kit was incomplete. The following items were missing: adhesive tape, cotton balls, scissors, thermometer, and tweezers.
