The parent handbook was missing a few things to include the promotion of indoor and outdoor activity.

Two caregivers did not have training in Understanding building and physical premises safety, including identification and protection from hazards that can cause bodily injury such as electrical hazards, bodies of water, and vehicular traffic; and Handling, storing, and disposing of hazardous materials including compliance with wearing gloves when handling blood or bodily fluids containing blood.

Based on information obtained from interviews with the operations staff, the victim?s parent and oberservations from video footage it was determined that a caregiver was observed rejecting a child by knocking their the water bottle from their hand and making the child go away when the child approached the caregiver after the child accidentally hit another child with their water bottle.

An infant was observed in crib that contained a noise machine. The infant was observed asleep on its on with a pacifier in its mouth that had a stuffed animal attached to it,

Children were not given two opportunities for outdoor play.

Attendance records were not available for the directors of the daycare center.

The standard was evaluated as part of an investigation and was found deficient. Based on information gathered and observations, it was determined that the operation was not following the meal patterns established by the U.S. Department of Agriculture (USDA) Child and Adult Care Food Program (CACFP) that is administered by the Texas Department of Agriculture. One child allergic to a fruit being served for lunch and snack was not provided an alternative food at lunch or an afternoon snack.

Four employees did not have Documentation showing how the employee meets the education qualifications. One employee did not have a statement signed and dated by the employee verifying the date the employee attended orientation.

One caregiver did not wash a toddler's hands after a diaper change.

One infant was in a restrictive device for more than 15 minutes.

Two infants under the age of 12 months were grouped together with children up to four years of age.

19 children were observed with one caregiver at 7:45 AM.

The Operation does not have a fire inspection conducted by the local fire marshall.

Two infants did not have written feeding instructions.

The operation did not have a current Gas Leak Inspection.

The written operational policies were missing a statement regarding infant safe sleep practices at the operation.
