The operation failed to notify parents of less serious injuries when the parents pick up their children from the child-care center.

A child was pulled through the hallway of the operation by the previous director.

The previous director failed to comply with standards by conducting prohibited punishments and not ensuring parents were notified according to standards.

Operation allowed staff member to be present at the operation without the correct background check for a staff worker present with children. Caregiver was present with children throughout the operation.

Director did not have attendance records or time sheets listing all days and hours worked.

Operation did not have an annual Environmental Health inspection.

Liability Insurance was not made available during inspection.

Operation did not have an annual Fire inspection.

Operation has not tested carbon monoxide detection system monthly.

Operational policies were not updated to the new standards.

Operation did not conduct a monthly drill in the month of October.

Food allergy emergency plan was not signed by a parent.

One caregiver was over ratio with 6 infants. Infant room had a specified age 14 months

At the time of inspection, the annual fire inspection had not been conducted for 2022. The last fire inspection conducted was dated 6/7/21.

At the time of inspection, 1 child with a noted food allergy did not have a food allergy plan on file.

Children observed walking around with no activities and not enough outdoor toys for children to play with.

Storage space was not clearly labeled with the child s name, a photograph of the child, or other symbol the child recognizes as his own.

Children ages 18-24 months and 2 years old were observed crawling under a play structure designed for ages 3 and up.

Lysol bottle was in the children's bathroom inside a cabinet unlocked and open.

Operation does not have food allergy emergency plan for an infant and toddler.

There were no storage for each child's belongs and storage area was not labeled for each child.

Children observed walking around with no activities and outdoor toys were not brought outside for children to play with

Water was not served or offer during snack in the infant room.

18-24 month old classroom did not have children alllergies posted.

Permit holder allowed caregiver to be the primary caregiver supervising children alone which is against the caregiver's provisional status.

A child was left outside for about 1-2 minutes on the playground.

The operation violated conditions listed on the employee's provisional eligibility letter. The employee was the sole caregiver and was left alone with a group of children.

One caregiver was supervising 6 infants in the age ratio 0-11 months. One caregiver was supervising 17 toddlers in the age ratio 18-24 months.

Permit holder allowed infant room to be out of ratio a second time and failed to correct the concern regarding ratio.

Director/ caregiver allowed an infant to sleep in a restrictive device (bouncer).

Permit holder allowed a caregiver to be present with infants directly after being informed by CCR Rep that all caregiver must receive an eligibility letter from BGC unit.

An operation allows a person to be present at the operation before receiving notice from the CBCU regarding the person's eligibility.

Caregiver was supervising 6 infants in the age ratio 12 -17 months.
