The monthly playground maintenance checklist was not available for review. This was corrected during the inspection when the checklist was completed.

One child does not have emergency care authorization.

The product certification was not available for review. The product certication was reviewed and signed during the inspection.

An outlet in the toddler classroom was uncovered. The director replaced the outlet cover during the inspection.

Four children do not have current immunization records.

Two caregivers hired after 9/1/23 did not have the Pre-Employment Affidavit form 2912.

During the course of the inspection, the inspector found that the director?s training certificates, first aid and cpr training for six staff members and the annual fire inspection were falsified.
