Expired food items were observed in the kitchen including tortillas and canned food. Note: This was corrected at inspection when provider removed the food items from the operation.

Antisepectic solution and wipes are expired. Note: This was corrected at inspection when provider brought a new first aid kit to the operation that has new antiseptic wipes.

Trash can in the toddler was uncovered, there were diapers inside the trash can. Note: This was corrected at inspection when trash can was replaced with one with a lid.

Toddler - Prek Classroom does not follow the daily schedule. Class remained indoors playing with blocks while schedule indicates children have outdoor time from 10-10:45AM. Infant Room does not have a written activity plan and daily schedule does not include outdoor play.

A staff member was present at operation and responsible for caring for children. Employee has not completed high school education or equivalent.

Eight children's files were reviewed. Four files are missing admission date, two emergency contact address, two health statements, four immunization records, three operational policies received statement and one file was not available for review at the time of inspection.

One out of four staff files reviewed is missing 12 hours of annual training.

Trash cans within the restrooms do not have lids.

Menu posted is dated 5-8 to 5-13. Per provider dates were entered incorrectly, however, food items served during lunch do not match menu. Note: This was corrected at inspection when provider updates dates and food items served.

Children's Products Certification posted was last reviewed / signed on 04/05/2020.

There were four children in the infant room, two children in the 4-5 room and 8 children napping in the toddler room. Only one staff member was present to supervise all classrooms.

One infant age child and three infant age children were asleep in high chairs.

A child was observing playing with a tablet during nap time.

Based on the information obtained during the course of the investigation, Director has failed to provide leadership and demonstrate an understand of Minimum Standards. This has resulted in high-risk deficiencies including safe sleep and supervision.

Based on the information obtained during the course of the investigation staff has not demonstrated competency to perform assigned duties such as providing a consistent positive environment for children and comforting young children.

Based on information obtained during the course the investigation operation has not maintained consistent staff attendance records.

Six out of twelve children napping were napping directly on the carpet. Mats were labeled with different children's names and solid color mats did not have a way to differentiate the floor from the napping side.

One out of five children's files reviewed is missing immunization records.

Two nails were observed in the playground area within reach of children. NOTE: This was corrected during inspection by the nails being covered.

Caregiver left two infants unsupervised to be in the kitchen, room next door, completing other duties. NOTE: This was corrected during inspection as caregiver returned to the infant classroom and needed supervision was achieved.

The youngest child in a classroom was 9 months old. The oldest child was 3 years old. There were more than 13 children at the operation during inspection. The 18 month difference in age was deficient. NOTE: This was corrected during inspection as 3 year old child was moved to another classroom. Child/Caregiver ratio was achieved.

Infant daily report is not being completed for two infants at the operation. Caregiver confirmed report is not being provided to parents.

A 10 month old infant was observed sleeping in a high chair. Infant had a blanket covering their body except the head as observed during inspection. NOTE: This was corrected during inspection as child was moved to their designated crib by caregiver.

Three caregiver records requested by Inspector were not available for review during inspection.

Written feeding instructions for two infants not ready for table food were not available for review.

Four school age children were observed watching a video of Happy Tree Friends not related to planned activities on a tablet. A school age child was playing a video game not related to planned activities in an electronic device. NOTE: This was corrected during inspection as electronic devices were removed by director.

Carbon monoxide detector was not present at the operation.
