Feeding instructions are needed for two infants enrolled.

Records were evaluated for eleven children enrolled and the following was determined: One child enrolled needed emergency care information such as physician/hospital name, address, and phone number. One child enrolled needed authorization for emergency medical treatment. Four children enrolled were missing the admission date. Three children enrolled need updated immunization records.

During today's inspection I observed two children ages 8 and 10 months old asleep in cribs. The children were covered with blankets. During today's inspection I observed a five month old child asleep in a crib. The child was asleep on two pillows as well as a boppy pillow. The child was also covered with a blanket and two toys were in the crib.

During today's inspection a five month old was provided a bottle while sleeping in a crib.

Upon arrival I entered the daycare room and observed a caregiver seated on one side and an infant asleep in a crib on the opposite side. A wall between them obstructed the caregivers view of the infant.

During the inspection the platform to a slide is cracked posing a safety hazard to children.

During the inspection one caregiver was left with 12 children. I observed 3 children who were under 17 months old and the remaining 9 children were between 20 months to 4 years old.

A box of disinfectant wipes, bottles hand sanitizer and a can of disinfectant spray were accessible to children during the inspection.

Feeding instructions for one infant had not been updated since February 2023.

The operational policies did not include the following information: 1. The duration of physical activity both indoor and outdoor 2. A plan to ensure physical activity occurs on days when extreme weather conditions prohibit or limit outdoor time.

1 out of 4 children's records evaluated did not contain immunization documentation.

2 out of 4 children's records evaluate did not contain special needs statements. Note: One child's special needs statement was completed during inspection.

Staff hired did not have documentation showing their hire date. El personal contratado no tenia documentacion que demostrara su fecha de contratacion.

During inspection two cribs were observed with loose fitting sheets. Noted: Pictures were taken. Durante la inspeccion se observaron dos cunas con laminas sueltas. Nota: Se tomaron fotografias.

Staff with birthdate 12/5/ 2002 is working at operation without completing a GED program and or high school diploma. El personal con fecha de nacimiento 12/5/ 2002 esta trabajando en la operacion sin completar un programa de GED y/o diploma de escuela secundaria.

During inspection staff hired on 6/6/2022 was observed working with children with a pending background check. Durante la inspeccion se observo personal contratado el 6/6/2022 trabajando con ni?os con una verificacion de antecedentes pendiente.

Staff hired on 1/1/2021 did not have proof of education on file. El personal contratado el 1/1/2021 no tenia prueba de educacion en el archivo.

Primary caregiver had no updated trainings available at the time of the inspection.

1 out 10 children did not have a immunization record on file to review at the time of the inspection.
