A child was observed using a tablet to watch Netflix. Note: This was corrected at inspection when the device was stored away.

One of the mats in the toddler classroom was not labeled with the childs name. NOTE: This was corrected at inspection when caregiver labeled the mat with the childs name.

In the outdoor area it was observed there was two small ant piles in the childrens play area.

During breakfast time; a four month old child was observed in a swing with a bottle. Note: This was corrected at inspection when the bottle was removed.

One out of three staff files were reviewed is missing the assessment tool for pre-service training.

Operation policies were reviewed and missing the following: -Physical and Outdoor activity procedurers -Emergeny Prepardness Plan

Daily schedule indicates one opportunity for outdoor play.

Two out of three staff files reviewed have foreign certificates and do not indicate education is equivalent to high school diploma/GED.

There is a play structure with two slides with a height of 27inches not surrounded by surfacing.

The Infant Daily Report was missing the section for Summary of Activities.

There is not a working carbon monoxide at the operation.

Evacuation and relocation diagram does not indicate the designated location inside the child-care center where all caregivers and children take shelter from threatening weather
