In review of employee records, four staff did not have proof of training in handling, storing, and disposing of hazardous materials.

Director did not have proof of training available for review in preventing and responding to emergencies due to food or an allergic reaction and emergency preparedness.

In review of personnel records, 5 employees did not have proof of training on hazardous material available for review.

In review of transportation binders/records, two children's documents did not include emergency contact phone numbers, This was corrected at inspection when director made copies of the emergency contact information, including phone numbers from the children's personal files and placed them in the transportation binder.

While inspection transportation vehicles, vehicle with plate SCC-2023 had a fire extinguisher with a service date of October 2020. This was corrected at inspection when the director replaced the fire extinguisher with one that has a service date of 08/2023.

The play structure observed to be used by one-year olds has a broken piece creating a pinch and/or shear point. This was corrected at inspection when director covered the cracked area with duct tape and made arrangements for maintenance to address issue.

In the infant room, three cribs had sheets that fit loosely.

On the outdoor play structure for school age children, two screws were missing a plastic covering, exposing the end of the screw. This can cause an injury to a child who slides their hand or arm across the screws.

During a van route the driver failed to conduct a name to face check against the list of children being transported and as a result a child that did not belong to the daycare was picked up and transported to the daycare.

During a van route the operation did not have the emergency contact information for a child that was transported.

The operation did not report an incident to Licensing that involved an unknown child been picked up and transported to the daycare and child was not enrolled at the daycare.
